MEMORANDUM ORDER NO. 220

CREATING AN INVESTIGATION COMMITTEE TO INVESTIGATE AND DETERMINE ALL THE FACTS AND CIRCUMSTANCES SURROUNDING THE ALLEGED FALSIFICATION OF PERSONAL DATA SHEET BY MRS. MERCEDES AGBAYANI PERALTA OF THE PRESIDENTIAL ACTION CENTER.

There is hereby created an Investigation Committee for the purpose of investigating Mrs. Mercedes Agbayani Peralta and deter mining all the facts and circumstances surrounding the alleged falsification of Personal Data Sheet.

The Committee shall be composed of:

For the purpose of the investigation, the Committee is hereby authorized to take testimony or receive evidence, which authority shall include the power to administer oaths, summon witnesses, and require the production of documents by subpoena duces tecum, pursuant to Book I, Section 37, of the Administrative Code of 1987.

This Committee shall submit its findings and recommendations as soon as practicable.
